CAD did a comic about one of his female leads having a miscarriage.

the internet never forgets and did an endless amount of edits for it.

the new trend, which the image in the OP is a part of, is to make an image that subtly mirror the comic.

they're getting crafty with it too

Why though?

It's a dumb funny gaming comic right? Was it something the author was going through or did he actually come up with the idea of putting a miscarriage strip in the comic by himself?

CAD gained infamy by being a cheap, terrible and unfunny webcomic that achieved popularity largely through being early to the punch. The graphics were 90% copy-paste jobs, the author was an egomaniac, the characters were pretty universally awful people and the comic itself forwarded terrible opinions. It kept on going well past its sell-by date (1995) but I think it's dead now. The main character was IIRC kind of a self-insert, but I don't think the miscarriage was based on a true story - just cheap drama.

Loss.jpg is probably its enduring legacy purely due to how spectacularly incompetent everything about it is. Look at it. Christ.

And even knowing now what the OP looks like, it bears no resemblance.
