I thought you had to do it on Normal, but I did it on easy and got the Extra. Though when I go to pick a character it still says I have to beat it on Normal for the others. I don't find PoDD that hard. I would've 1cc'd Easy on my second try if it hadn't been for me dying to the final boss at pretty much the last minute. Though maybe it has a steep difficulty curve.
