There's actually a funny story behind that. When Genndy Wars was given General Grievous to use for the show, they weren't given the finalized version of the character. They didn't know about the cough or that George Lucas planned for him to be something akin to a Saturday Morning Cartoon villain; they only knew a rough sketch of his appearance and that he's a lightsaber-wielding cyborg known as the Knightslayer who takes the lightsabers of every Jedi that he kills. So, they figured, "Well, he must be a badass then."

When the final season was being made, they were told everything in order to tie in the final season to Revenge of the Sith, and they realized how different their version was from the one in episode 3, so they came up with him getting his chest Force-crushed by Mace Windu as an explanation for the difference. Honestly, I thought it was a good explanation that even elevates Episode 3 Grievous, as it means he put up a decent fight against Obi-Wan while still crippled from what Windu did to him. 

Audiences liked Genndy's version of Grievous so much that every other Clone Wars media until The Clone Wars Cartoon used Genndy's badass version of Grievous, and I honestly wish the cartoon had gone with that version as well, instead of the pitiful excuse that gets beaten by Gungans
